Institutional Camp/Clinic Approval Form
This form must be completed if ANY Dartmouth athletics staff member wants to own, operate, or conduct a camp/clinic, either on- or off-campus in which prospective-aged student-athletes may participate.
Post Camp/Clinic Employee Form
This form must be completed after every institutional camp/clinic conducted in which there were employees.
Non-Institutional Camp/Clinic Approval Form
This form must be completed if you will be employed, in any capacity, by a camp/clinic in which the majority owner (> 50%) is someone other then a Dartmouth athletics staff member.Â

Practice Logs - *Be sure to use a form with your specific declared week. The forms below are designed with a declared week of Sunday-Saturday. If your week is not Sun-Sat, see the Compliance Office for the correct practice log.
Individual Practice Log - Uesful when not all team members conduct the same activity on the same day.
Team Practice Log - Useful when every team member conducts the same activity each day.
Non-Championship Segment - Only the following sports have a non-championship segment where practice hour rules are different than when they are in their championship segment; Baseball, Field Hockey, Lacrosse, Soccer, Softball, and Volleyball.
